TRIAL CLASS SIGN UP FORM

If you would like your child to Trial a class before enrolling into class/es, then please fill out this form.   If you would like to register for classes, please create an account and then you will be directed to the parent portal to select classes and register for the season.  Please note, Contemporary Ballet Dallas cannot hold season enrollment into any class for trial students.  If the class is full, then trials will not be allowed for that Day and time of the class and other options will be provided. If the class enrollment is full upon the Trial student wanting to enroll after Trialing a class, then other options will be provided. Please fill out this form below.
